Your first visit to our Fort Worth office
A relaxed, thorough first appointment with clear answers. Here is what to expect, what to bring and how to get ready.

Four simple steps
- Book and fill out forms
Call or request a time online. We send secure forms so you can skip the clipboard.
- Talk with us first
Before any exam, we ask what is bothering you and what you want from your smile.
- A complete exam
Pocket charting, digital x-rays, photos and a screening for oral cancer.
- A clear, written plan
We walk through findings and give you options, priorities and costs to take home.
Checklist
- Photo ID
- Insurance card (if you have one)
- Medication and allergy list
- Recent x-rays from another dentist
- Referral slip if your dentist sent one
Paperwork before you arrive
After you book, we text or email a link to secure new patient forms. Completing them ahead of time shortens your check-in and gives the doctor time to review your health history.

What should I bring to my first visit?
Bring a photo ID, your dental insurance card if you have one, a list of your medications and any recent x-rays from another office. If you completed your forms online, there is nothing else to fill out when you arrive.
How long is a first appointment?
Plan for about 60 to 90 minutes. That gives us time for a full gum evaluation, digital x-rays, an oral cancer screening, a conversation about your goals and a written treatment plan before you leave.
Can I get a virtual consultation first?
Yes. Send us a few smile photos and a note about your goals, and our team will reply with a personalized recommendation and preview. It is a simple way to explore cosmetic or restorative options before coming in.
I'm nervous about the dentist. What can you do?
Tell us when you book. We can schedule extra time, explain each step before we do it and offer nitrous oxide, oral sedation or IV sedation for longer procedures. Many anxious patients tell us their first visit was easier than expected.
Let's get you on the schedule.
Call our Hulen Street office or request a time online. We will review your insurance, explain every option in plain language and give you a written plan before anything starts.
